I know there has already been a long and lengthy discussion over the skin forms. So I want to keep my question simple.
Do kids who had to have a skin form for Subs for ringworm, etc. need to have a new skin form from the Dr for Districts and State or are they fine if they bring the one from Subs?

That is not the way I understood Mr.Treaster's post on this under topic Skin form expiration he said that the expiration date on the form would be followed and that there was no 7 day or 3 day rule.

Also note that the By-law regarding the skin form doesn't say ANYTHING about how many times it needs updating per year, month, or week, or for any non-communicable conditions either - I'm going to get a new form WITH the letterhead just to be on the safe side - They definitely need to address this in the next state meeting - very confusing

I agree it needs to be addressed but according to Mr. Treaster a new form is not needed and he also says that the letterhead is in the by-laws but not inforced. To be on the safe side I would get the letterhead so not to ruin your childs chance to make it to state because of the confusion over this by-law. The letterhead by-law needs to be gotten rid of. My Dr. Office woldn't hardly give me anything with letterhead on it because it is against office policy my wife had to sweet talk them into it, she is better with words than I am.
