ž	Charter your Club
	*	Complete “Application for Club Charter.”	$50.00
	*	Complete “Club Address and Officer List.” This should be done at the same time as the club charter.
	*	If you wish to add the owner/operator of your practice facility as a Certificate Holder, complete the section Certificate of Insurance and Fees on the “Application for Club Charter” form.	$20.00
	*	If you added a Certificate Holder, and want that Certificate Holder shown as an Additional Insured on your liability insurance policy, complete the “Application to Add Certificate Holder as an Additional Insured” form. Make additional copies of this blank form first if you will be requesting multiple Additional Insured (usually for a tournament facility or a second practice facility).	Free
ž	Register your Members
	*	Returning Members: Use the form that lists all members previously registered with your club. This form includes anyone who has been registered with your club in the past 30 months. Circle or highlight the coaches and wrestlers you wish to register.	Competitor cards are $30.00, Coaches cards are $35.00
	*	New Members: Use the “Individual Membership Registration” form. Please type or print legibly on this form!
	*	Members transferring from other clubs: If you have a coach or wrestler who is new to your club, but has been a member of USA Wrestling with another club in the past, include him/her on the “Individual Membership Registration,” but please also make a note of this fact on the form. This helps avoid duplication of records in our database.
ž	Sanction your Tournament
	*	Complete the “Application for Sanction of Event.”	$35.00
	*	If you wish to add the owner/operator of your tournament facility as a Certificate Holder, complete the section Certificate of Insurance/Publicity Information on the “Application for Sanction of Event” form.	Free
	*	If you have a Certificate Holder you want shown as an Additional Insured on your liability insurance policy, complete the “Application to Add Certificate Holder as an Additional Insured” form. If you have already requested an additional insured with your club charter, a separate form is required for an additional insured on the sanction.	Free
