Coach Fretwell will walk you through the fundamental technique and philosophy behind all scoring options from each position and counter attacks. Please provide specific written directions for any medical needs that you deem necessary while attending this camp. I understand that I a required to maintain and carry accidental insurance coverage for the named youth on this application and verify coverage information attached herewith is true.
I verify that my son/ daughter has been checked by a licensed physician and is physically able to participate in this camp. I herby authorize Jaguar Wrestling and CMP CAMPS to act for me to their best judgment in any medical emergency, while they are attempting to contact me. I waive and release this camp from any and all liability, injury or illness incurred while attending this camp. The camper while attending this camp will use facilities at their own risk. The camp and it staff shall not be liable for any damages that arise from personnel injury sustained by the camper on the way to, during and on the way home from the camp. The camper his/her parents accept full responsibility for any damages or injuries that occur during this time frame, and so hereby fully and forever, excuse and discharge the camp staff, owners, employee’s, Desoto School District and its agents from any and all claims, demands or rights of action or cause of action, present or future, whether the camper known, anticipated or unanticipated, resulting from or arising our camper’s participation in the Jaguar Wrestling CMP CAMPS and in the use of facilities.
